All-call forwarding (sometimes called unconditional or immediate forwarding) sends every incoming call straight to the destination. Your desk phone does not ring at all. This is what you want when the office is closed or you want an outside service to handle everything for a while.
Conditional forwarding only redirects calls in certain situations. The common conditions are: no answer (the line rings a set number of times, then forwards), busy (you are already on a call), and unreachable (the phone is off or out of service). Conditional forwarding lets your line ring normally first, which is a common setup for after-hours or overflow coverage.
One thing to know: all-call forwarding usually overrides voicemail on the original line, and on some systems it can override hunt groups that would otherwise pass a call to other extensions. If you rely on either of those, test carefully after activating.

Activation Codes by Provider
The code you use depends on your exact service, not just the brand name. AT&T wireless, AT&T Collaborate, and AT&T Phone for Business all use different codes, for example. Identify the service name on your bill or account portal first, then use the matching instructions below. These are the procedures each provider has published for its own service.
All-call (unconditional) forwarding: Verizon mobile uses *72 plus the 10-digit destination, set up from the phone while in Verizon coverage, and *73 disables; Verizon business landline also uses *72 and *73 from the same line. T-Mobile uses **21*1 then the destination and #, disabled with ##21# (##004# resets to defaults). AT&T Phone for Business uses *72, destination, #, with *73# to turn it off; AT&T Collaborate uses *72 plus destination and *73 to disable. Comcast Managed Voice (Panasonic TPA65) uses lift handset, *72 and destination, then hang up, with *73 to disable. Optimum Business Phone uses *72, wait for dial tone, then the destination, with *73 to turn it off. Frontier Unlimited Digital Voice uses *72, wait for the second dial tone, then the destination, with *73 to disable. C Spire uses *72 plus the 10-digit number and Call, with *73 then Call to disable.
Conditional (busy or no-answer) forwarding: Verizon mobile uses *71 plus the destination for unanswered calls. T-Mobile uses **61*1 for no reply, **62*1 for unreachable, and **67*1 for busy, each disabled with ##61#, ##62#, or ##67#. AT&T Phone for Business uses *92 for no answer and *90 for busy; AT&T Collaborate uses *92 for no-answer and *62 for busy. Comcast Managed Voice uses *92 for no-answer and *90 for busy. C Spire has not published conditional codes, so confirm any no-answer feature with them directly rather than borrowing another carrier's codes.
Portal-based setup: Vonage Business Communications lets you enable Call Forwarding in account settings and choose Forward Calls to a phone number; verify with an outside test call. Spectrum Business Voice uses SpectrumBusiness.net: open Voice, choose the number, then Voice Settings or Forwarding Manager, and pick all calls or busy/no-answer with a destination.

Forwarding to a Cell Phone or an Answering Service

Will callers know my calls are being forwarded?
No. Callers dial your normal business number and the forwarding happens on the network side. They only notice if the destination answers differently, such as a cell phone greeting or a receptionist.
Does forwarding work with texts?
Generally no. Carrier forwarding features documented here cover calls, not text messages. Verizon, for example, states its forwarding applies to calls only.
Can I forward only after-hours calls?
